To apply for a space in Kids’ Delight academy, this form should be completed and returned with a copy of the child’s passport or birth certificate, two (2) passport size photographs and a non-refundable fee of GMD1000.00(One Thousand Dalasi).

Kids Delight Daycare — Enrollment Agreement

1. You hereby agree to formally enroll your child at Kids Delight Daycare located in our premises, effective from the agreed date.

2. By signing this agreement, you confirm that:
(a) all holders of parental responsibility for the child have signed this agreement and that no one else holds parental responsibility for him/her.

3. The payment for the services will be paid in the amount stated by the Daycare. The payment can be made either in person or transferred to the account provided by the administration.

Acceptance and Deposit

1. An offer of a place for your child at the Daycare is accepted by submitting the signed agreement and paying the deposit.

2. The deposit is not refundable if your child does not take up their place at the Daycare. The limited exception to this is where the Daycare actually fills the specific vacancy created by your child's withdrawal, in which case the Daycare shall refund the deposit to you.

Notice Requirements

1. If you wish to withdraw your child from the School, you must give a clear term's notice in writing.

Our Obligation

1. While your child remains a pupil of the Daycare, we will exercise reasonable skill and care in respect of his/her education and welfare. This obligation applies during the normal operation hours and at other times when your child is permitted to be on the School premises.

2. If your child requires urgent medical attention while under the Daycare's care, we will, if practicable, try to contact you to obtain your prior consent. If not practicable, we will make the decision on your behalf if consent is required for urgent treatment recommended by a doctor or other medical practitioner. In case of such emergencies, an on-call nurse is present at the daycare during operation hours.

Ending the Contract

1. The Daycare may end this contract at any time by notice in writing to you, without any obligation to return any deposit or fees paid to you, if:
(i) you do not make a payment to us when it is due and you still do not make payment within fourteen (14) days of us reminding you that such payment is due.
